feat: 体重管理异常事件页面补全，新增历史干预模块及个人目标详情
- exception-system-dirty.html: 补全系统脏数据页（筛选栏/操作栏/9列表格/分页/10条Mock数据）
- exception-result-abnormal.html: 补全结果异常页（筛选栏含单位部门联动/操作栏/9列表格/分页/10条Mock数据）
- 新增 history-plan-list.html、history-plan-detail.html、personal-target-detail.html
- activity-statistics.html、weight-ranking.html、detail-data.html、personal-target-data.html 等页面大幅重构
- 全部36个页面顶部导航新增「历史干预」菜单
- index.html SITE_MAP 同步更新
- health-monitor/exception-result.html 移除异常等级筛选及列
- CHANGELOG 追加 2026-06-18 条目
